the scientific method of investigation, as applied to drug therapy selection. Dr. McPherson will
identify the “level of concern” when transitioning a patient with serious illness from the hospital
to the community and discuss benefits and barriers to transitions between Inpatient hospice,
Home-based hospice, LTC/hospice, Palliative Care LTC and Palliative Care Home.
Additionally, participants will learn to develop best practices for transitions in care and
medication management from hospital to the community in patients with advanced disease
processes. The best practices reviewed will include factoring consideration of formulary
restraints, familial attachment to medications/monitoring, thinking ahead - availability of
medications, changing routes of administration, misuse/abuse/diversion of medications, facility
CRAZY regulation, goals of care and levels of care, communication issues and barriers,
medication myths, misconceptions, beliefs, cost of medications including copays, cultural and
healthcare beliefs/values and medication titration and monitoring.
